Accredited Hospices of America in Sugar Land cares for patients at whatever place they call home . . .
We give care in whatever setting is requested by each patient. If you need care in your own home, an assisted living facility, a skilled nursing facility, or a hospital, we will give you medical support. If you need care outside of your home, our admissions nurse can find a facilities partner with Accredited Hospices of America for you.
In-Home
Many hospice patients choose to be cared for in their own homes. Each patient and his or her family work with our primary care nurse and your primary care physician to create a care plan.
Skilled Nursing Facility or Assisted Living Facility
Many hospice patients live in skilled nursing or assisted living facilities. Wherever you live, the visits by our hospice caregivers are arranged just the same as they are for patients in private residences. Our team works with staff at any facility to create a care plan for each patient’s needs.
